What to Send
The best way to ensure your prescription includes all the information we’ll need is to send us an e-script and fax using the Prescription Referral Form with the supporting documentation requested within the below Prescription Referral Checklist. Our team will match the e-script to the supporting documents and reach out if we have any questions.

Biologics Prescription Referral Checklist
- The prescription and explicit start date
- A written diagnosis, primary diagnosis code (ICD-10) and stage of disease for the prescribed medication
- Clear dosing instructions: total dose, quantity, frequency (qd, bid) and refills
- A description of the patient, including:
- Height and weight
- Date of birth
- Mobile phone and caregiver contact information
- Copies of the front and back of all insurance cards, including prescription and medical, and demographic fact sheet, including the patient email
- List of all current medications
- Clinical information that supports the treatment decision, including pathology or laboratory reports, treatment history, and a description of the treatment plan
- For Nurse Practitioners or Physician Assistants, please include the name, NPI# and DEA# of the supervising physician
- Patient navigator/office contact(s) information
What Happens Next?
- Our team at Biologics will process your patient’s prescription and fax your practice to confirm and verify all information.
- From there we advocate for your patient through the benefits investigation, prior authorization and financial assistance processes.
- Want status updates? The pharmacy will send updates to your practice when the prescription is processed. For those patients that provide an email address, a notification with a tracking number may be shared.
