How do patients sign-up for a personalized care plan?

Our cross functional clinical team identifies high-risk patients receiving specific therapies. To confirm the need for a personalized care plan, a pharmacist first conducts a risk assessment to identify any barriers to treatment before receiving therapy. We pair every patient with a dedicated nurse who will manage their care during therapy. Our Risk-Based Care plan is part of our way of helping patients achieve the best possible outcomes.

Do my patients need a personalized care plan to receive their prescription from Biologics?

No, patients do not need a personalized care plan to receive their prescription from Biologics. However, if your high-risk patient qualifies for the risk based care program, they will receive a phone call from a pharmacist to assess their potential barriers to treatment. If a patient does not want a nurse to follow-up, they can opt out of the program and still receive their prescription.

Will a nurse keep me informed of my patient's progress?

Our Risk-Based Care program closely models the type of care that you receive in face-to-face clinical settings, where patients are supported by familiar providers who know the particulars of their situation. This care model recognizes that patients benefit from continual, consistent and individualized support at a level that’s appropriate for them throughout the treatment journey. A nurse will only contact a patient’s doctor, if the patient makes this request.

How often will a nurse contact a patient?

The cadence depends on a patient’s personal challenges, clinical needs and the ease, or difficulty, of therapy. Sometimes, the frequency of calls to a single patient change throughout the course of therapy. Supporting each patient with an individualized plan allows nurses to manage care more effectively—the way it’s done in a hospital or clinic setting.
